New Patient Registration

consultation1The doctors welcome new patients who live within our practice area. 

You may register as a patient by calling in at the surgery of your choice and completing the necessary forms. You will be asked to provide some forms of I.D (listed below), and when we have all these documents, you then need to make an appointment to attend for a new patient medical.

Suitable I.D = Passport OR Driving Licence, if not able to provide these then - birth certificate, National Insurance Card

Suitable Proof of Address - Recent utility bill (not telephone), council letter, recent bank statement.
