Online New Patient Registration With The Practice


Before you apply to register, please can you ensure that your address is within our practice catchment area using the tool below.

To register with our practice, all patients are required to complete BOTH of the forms below (Form 1 and Form 2), by following the links provided.

(We are aware that the New Patient Health Questionnaire currently states it is for adults, but we DO require this completing for children also)

Once you have submitted your registration forms, please attend the surgery at your earliest convenience to provide proof of identity i.e. photo ID and proof of address. We will also ask you to check and sign your application forms.  Once you have provided proof of ID, we will then be able to accept and process your registration.

Once you have been accepted as a patient, your medical records will be transferred to the practice. Your medical records may take several weeks to arrive at the practice.

The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case, simply choose another GP in your local area.
