New Patient Registration
The doctors welcome any new patients who live within our practice area.
You will need to complete new patient registration forms which are available at reception or alternatively can be completed via our website under 'Registering Online'.
We will ask you for proof of identity i.e. photo ID and proof of address.
- Driving Licence
- Birth Certificate
- National Insurance Card
Suitable Proof of Address:
- Recent* utility bill (not telephone)
- council letter
- recent* bank statement.
*Within the last 3 months
Once you have been accepted as a patient, your medical records will be transferred to us from your previous practice. Your medical records may take several weeks to arrive with us.
The practice has a non-discrimination policy for accepting new patients. There are a number of reasons why you may not be able to register with your chosen GP. For example, you may live too far away. If this is the case, simply choose another GP in your local area.
