Reception and Admin Team

Receptionists provide an important link for patients with the practice and are your initial contact point for general enquiries. They can provide basic information on services and results and direct you to the right person depending on your health issue or query. Receptionists make most of the patient appointments with the GPs and nurses. They also perform other important tasks such as issuing repeat prescriptions and dealing with prescription enquiries, dealing with dealing with patient records and carrying out searches and practice audits.

Care Co-Ordinator

Our Care Coordinator helps ensure patients receive the right care and support by coordinating services across the healthcare system. They work closely with GPs, nurses, and other healthcare professionals to help organise and manage patient care, particularly for those with complex or long-term health needs. The Care Coordinator supports patients by helping arrange appointments, follow up on care plans, and ensure communication between different services runs smoothly. They may also assist patients in accessing community and support services when needed. By helping to organise and streamline care, the Care Coordinator plays an important role in making sure patients receive timely, well-coordinated support tailored to their needs.

Mental Health Worker

Our Mental Health Practitioner provides support for patients experiencing a range of mental health concerns. They work closely with the GP team to help assess mental health needs, offer guidance, and connect patients with the most appropriate services. The practitioner can provide support for issues such as anxiety, low mood, stress, and other emotional wellbeing concerns. They help patients access local mental health services, offer advice on self-care strategies, and support individuals in managing their mental health more effectively. Working as part of the wider practice team, the Mental Health Practitioner helps ensure patients receive timely support and are guided to the right care for their needs.

Pharmacy Team

Our pharmacy team plays an important role in supporting patients to get the most from their medicines and improve their overall health. The team consists of two Clinical Pharmacists and two Pharmacy Technicians who work closely with the GPs, nurses, and other practice staff. Our Clinical Pharmacists are highly trained healthcare professionals who specialise in medicines and prescribing. They carry out medication reviews, help manage long-term conditions, provide advice about medicines, and ensure treatments are safe and effective. They may also prescribe medications where appropriate and support patients with any medication-related questions. Our Pharmacy Technicians support the safe and efficient management of prescriptions and medicines within the practice. They assist with medication queries, prescription processing, and help ensure patients receive the correct medicines in a timely manner. Together, the pharmacy team works to ensure that patients receive the best possible care, safe prescribing, and clear advice about their medications.

Social Prescribers

Our Social Prescribers help support patients with non-medical issues that may be affecting their health and wellbeing. They work closely with patients to understand their individual needs and connect them with local services, community groups, and support organisations. Social Prescribers can assist with a wide range of concerns such as loneliness, financial difficulties, housing issues, employment support, and access to community activities. By linking patients with the right resources, they help people improve their overall wellbeing and quality of life. Working as part of the wider practice team, Social Prescribers focus on a holistic approach to health, helping patients access practical and emotional support within their community.
